Effective of May 1, 2009, Kurth Chiropractic had been accepted into 7 NEW Insurance programs. Because we are new to these, all patients that belong to any of these programs and wish to have us start billing them MUST fill out an Insurance Verification form. Additionally, please provide a 2009 or up to date insurance card. Please make sure to ask for this form; do not just assume we have this done already. We look forward to our new programs and know that this will help us grow our practice even more. Please help in doing this by referring your family and friends, and remember that we offer a referral reward program! If you have a Care Plan, we will be more than happy to schedule a time to sit down with you and your spouse to go over all the options available for you and your family. Please have patience with this as we will be quite busy with other patients too.
